BEVERLY MULLINS MEMORIAL SCHOLARSHIP

Deadline for submission: March 31, 2009 at 5:00pm (Extended 2 weeks from original deadline). Applications will be reviewed once a year during spring semester. Selections will be made by members of the Mullins? family and supported by the UC Berkeley Re-entry Alumni Association and UC Berkeley Re-entry Staff. You must have a 3.0 cumulative GPA to apply.

Steps to Apply for this Scholarship:

? Write a one page introduction that provides information about yourself and your child(ren).

? Write a short essay no more than two pages that describes up to three activities from the last 10 years in which you demonstrated significant leadership and overcame obstacles either through your work, school, or family life. Explain in detail what you did, what was accomplished and how your leadership made a difference.

o Submit your introduction and descriptive essay (maximum 3 page) on white paper, typed and double-spaced. At the top of each page, print your full legal name, Student Identification Number, and birth-date.

? If you are a Cal applicant, please attach a copy of your admissions application, or your on-line application summary, including your personal statement. (If you are a current Cal student, please skip this step.)

? Please provide documentation of financial need, e.g. a copy of the official Financial Aid statement showing the amount of family contribution required by you.

? You will need to be available for an interview in late April. Applicants outside the Bay Area will have phone interviews. If you receive the award, you will be required to attend the Re-entry Club?s Annual Benefit as a guest speaker in or around December of the following year.

? Mail the above documents to The Beverly Mullins Memorial Scholarship, Attn: Kristarae Flynn 1 Alumni House, Berkeley, CA 94720. Your mailing must be postmarked no later than March 31st.

? Do not send letters of recommendation or transcripts.
